No, you do not need to have an Apple Watch app in order to run Live Activities on Apple Watch. Live activities from your iOS app will automatically appear in the smart stack on Apple Watch, even if you don't have a watchOS app. This is made possible by leveraging the work you've already done to support the dynamic island on iOS. However, if you do have a watch app, you can opt to open it from a tap on the live activity in the smart stack, providing a more integrated experience.
